Tier 1 Dependant queries urgent

hi there,

I am stuck in few sections on Tier 1 Dependant(Extension) form and require some clarification.

In section K5 to K7, I have been asked UK Clearance Entry reference. I am unsure as to what reference numbers need to be provided in this section as in when I first entered UK in 2007 in work permit or when I filed the Tier 2 dependant Visa in 2010 followed by Tier 1 in 2011.

The Question is-

K7; What is the reference number of the visa or entry clearance?

I have looked in to the document guidance notes and they have not specified whether their question is specific to the dependant visa application or the UK Entry Clearance in General.

Similarly, in later sections I am being asked -

K12; What was your ticket booking or reference number?

For these the documents guidance clearly asks about the last entry in the UK.

Could someone pls urgently help with this ?
