Tier 2 - ILR (Permanent Resident) Eligibility

Hi,
I need a clarification whether I am eligible for ILR (Permanent Resident), I came to UK on Tier 2 ICT in Nov 2009 and did my first extension from India in sept 2010 and retuned UK 1st week of Sept. (I went India late June 2010 on vacation for my marriage, so I did my extension there even though I am still working in UK, and my salary is processed in UK, reason for delay in coming back is my visa got rejected twice, first time due to expired COS by company, second time missing bank statement). Recently did my extension in April 2013 which is valid till May 2014. If I further extend till Nov 2014 I will be completing 5 years in UK, am I eligible for ILR that is Permanent Resident. Appreciate if any clarifies my query, since I am getting different answer from every one.

Format the dates properly line wise rather than in a paragraph. Also provide exact dates rather than months and year to get a bit of clarity

As requsted, please find in requested format

Type = Visa Tier 2 (INT COM TRAN) MIGRANT / Valid from 04/11/2009 until 08/10/2010

Type = Visa Tier 2 (ICT) ESTABLISHED STAFF / Valid from 27/08/2010 until 30/04/2012 (This is extension I did it in Inda)

Type = T2 ICT LONG T ERM LEAVE TO REMAIN / Valid from 19/06/2012 until 14/05/2014

Based on above visa details can someone clarify below queries, appreciate your help.
1) Whether am I eligible for ILR (Permanent Resident) in Nov 2014?
2) Whether am I eligible to switch over to Tier 2 General, I have A rated sponsor who is ready to sponsor.
