Tier 1 Dependent Extension

Hello Everyone,
I am applying for Tier 1 partner Extension in 2 weeks time, along with Main applicant.
I have 2 questions
1> I have had a BRP before i moved to Tier 1 dependent in 2010. I never returned it. Now, in Section H of application, it asks for BRP info.
Do i have to to specify my BRP ,which i had before current Tier 1 dependent status ?( I looked at help and it says current BRP). i may be stupid to ask this, but here i am :-)
2> For Section I,i have lived at more than 2 addresses. What does extra sheet here means? Is it just a blank A4 sheet i have to add with old addresses in it?
