If any such children are applying as the
dependant of a Tier 1, 2 or 5 migrant as
their dependant, we would expect the Tier
1, 2 or 5 migrant to be with them when their
biometrics are recorded. If not, the person
accompanying them must be a responsible
adult aged 18 or over who is either the child’s
parent or guardian, or a person who for the
time being takes responsibility for the child.
Any such person must be named on the
application form.

in the last part, it said this person must be named in the form....i could not find any place that i can mention baby's mother name in the form? or the person must be named does not including the parents?
