Sorry I have an urgent question as I have PEO appointment tomorrow.

When completing Tier 1 G form, should I fill J9 section asking the details for verifications?

If so, what details do they need?

I believe you need to put details of your employer address, contact number.
Also tell your employer in advance that they might receive call from UKBA in order to check your employment status.

If you are self employed, then I dont know, may be you have to provide your accountant details.
