Help in completing Form MN1 for my 11month old son

Help in completing Form MN1 for my 11month old son

Hello All,

My son was born in the UK in 2012 and I was granted my ILR in February 2013 (last month). I'm in the process of registering my son as a British Citizen but there are some information which seem unclear and I'll appreciate if you could take your time to help.
Below are the queries and my questions are in italics.

SECTION 1:
"1.2 : Immigration and Nationality Directorate or Border and Immigration Agency of UK Border Agency reference number "

Do I leave this blank? Where can I get this information from?



"1.30 : Child's mother's full name & maiden name"


My passport still carries my maiden name as well as my Biometric Residence Permit; but my all other documents (NI, Bills etc) carry my married name. Do I write my married name and maiden name (e.g. Ann Pearsley; Rogers) or just Ann Rogers as it is written in passport?



SECTION 2: RESIDENCE REQUIREMENTS

2.1 : Date of child's first arrival?

Should I leave it blank?


2.2 : Full details of child's addresses in the UK for the last 5 years

Do i fill this?


SECTION 3: PARENTS' RESIDENCE IN THE UK.


Should I leave this section blank? My husband and I migrated from Africa before we were granted our ILR.




SECTION 5: REFEREES AND IDENTITY

Do I staple his passport picture to the form or place it in an envelope and staple the envelope to the form?



Is it compulsory for me to complete this section as my son is still a baby?


There are quite a lot of questions but I will really appreciate it if anyone can help me with this. I need to submit the form by next week.

In addition, what documents can I add to my son's birth certificate in the application? I hear it takes about 6 months to process the application via postal application and I am worried I may need my passport during the waiting period. Should I send the HO letter and a photocopy of my Biometric Residence Permit (BRP) along with the application? That is:

Child's Birth Certificate
HO Letter of ILR granted
Photocopy of passport
Photocopy of BRP
