ILR - Dependent application form

Hi,

I am on Tier-2 General visa and becoming eligible for applying for ILR in April ' 13. I understand I have to use Set (O) form for myself. What form should I use for applying for my wife with me as dependent? Should she be included in the same form as mine and separate form is not required?
