Help filling Tier 2 General Form

I have 3 issues filling up Tier 2 Skilled Worker form as:

1) In tier 2 skilled worker self assessment form in Part 8 point 8.5 asks about grade, would it be fine to put DISTINCTION in there, as my degree certificate says so.

2) Would only bank statement form UK based bank be fine? as I am applying from outside UK. Or do I need to get any other letter from Bank.

3) At the moment I have following documents:
a) CoS
b) Degree Certificate from UK university deemed by UK NARIC
c) Bank Statement covering last 90 days period
Do I need to add any other documents with it, just to support my
application?
