Can some body please share me the letter format by self, which would state the personal and serious and compelling reasons

Why do you need a letter format for this? Just briefly explain what ever your personal ,serious and compelling reasons are? You need to enclose supporting documentation with this brief note.Each one's compelling reasons are not same.
