what application form I have to submit?

Hi there,

My husband is on Tier 1 General visa right now and he is applying for his ILR July 2013.
I am on Tier 1 General Partner visa and have to apply for extension a few days after that he has got his ILR.
would you please let me know what application form I have to submit? I hear different things from UKBA and advisors.
