Form AN: Section 1: Q. 1.7 Maiden name?

Post by nnj10 » Sun Apr 28, 2013 6:39 pm

Q 1.7 Name at birth if different from above. (If the names you have given are different or spelt differently from the name shown on your passport, please explain why on page 13)

In section 1.5 surname and in section 1.6 first and middle names are provided but those are the names after name change due to marriage, and as it appears on current passport.

Are we expected to provide the maiden FULL name in answer to Q. 1.7?
Of course the last name and middle name is now different (husband's surname and first name respectively), but the First name (both before and after marriage is still the same).
Information and/or advice provided by me is of general nature and is not intended to substitute for informed professional legal or other professional advice.

I think you should definitely write your whole name at birth, because what you wrote (red) is not at all obvious (to me)!
