PBS Dependant Application Form Query

Hello,

I'm filling up the dependant form and have a few queries.

Section K7; What is the reference number of the visa or entry clearance?

The dependant obtained a student visa in 2005, got LTR under PSW in 2008 and then obtained a dependant visa in 2010 and is currently under LTR as Tier 1 dependant. Should the last dependant visa obtained by the dependant in 2010 be mentioned here or the first ever student visa in 2006?

K11; At which port did you enter the UK?

Should this be the last entry into the UK or any other entry such as first entry into the UK for example?

I would really appreciate any help in this matter.
