Tier 2 Application from Pakistan

Has anyone recently applied for a Tier 2 from Pakistan? Please can you advise what the process is. I was reading through the UKBA website but it seems to be slightly confusing with different regulations for each country.

I understand we have to submit an online application followed by an appointment at the visa centre? Is this correct. Also, what is the turn-around time?

Also - if someone is in the UK on a visit visa, is it ok if we submit the online application from here or would it be a problem when they see the entry / exit stamps?

Secondly, if the applicant has spent a few months every year in the UK visiting family (and applying for jobs / interviews) but not violated any visit visa rules, do they still look at it with concern?
