ILR dependent form clarification

Hi –
I need some clarification on my dependent ILR.

I have got my ILR and planning to apply for my dependents.

I got my ILR through WP + Tier 2 – ICT route.

Which form I should use for my dependent ILR?

My dependents were in India for 1 year 2011-2012 mid for my 2nd daughter delivery. I can provide medical reason letter for the absence from UK.

Please let me know.
