ILR for Wife Need help

Dear All Guru,s
I have got my ILr in FEB and now need to apply for my wife,s ilr as she will be completing her 2 years of residency in UK . She is on tier2 dependent visa right now valid till NOV 2014.My questions are :

1). what form I need to fill ?
2). what documents are required ?
3). Is same day service best or we can just post it , as its straight forward case.
4). Do she need some financial documents from my side as well (P60 or salary slips. ).
5). Do we have to maintain any balance in account.

There shouldn't be an issue just explain the mix-up when you arrive, make sure your case is well prepared e.g. 2 years cohabitation proof etc.
