Tier 1 General dependent in UK

Hi,
I am applying for tier 1 general ext visa for my dependent together with my extension. I have the following documents

1. Passport, and 2 photographs
2. Proof addres, ( council tax, NI)
3. maintainance ( my own statement )
4. Marriage certificate as per UKBA guidelines


Is there anything else i require for dependend.?

May I ask, why do you need the address proof?

Rest is all OK.

This is just to prove that we live at the same address.

It does not ask anywhere, but i have just added it as an additional document.
I am using the same PBS dependend application form which is the same for Tier 2 and Tier 4. I believe that is the correct form, or is there a seperate form for dependend extension?
