ILR - Absences Letter

Hi there,

I am going to apply ILR application in few weeks. I have make 5 family visit during my Tier 1 (G) duration and total no. of absence was 125 days in last five year and long stay was 51 days.

DO I STILL NEED TO PROVIDE THE ABSENCE LETTER.

I HAVE GO THROUGH THE FOURMS BUT COULD NOT FIND OUT THE ABSENCE LETTER. CAN ANYONE PM OR FORWORD THE LINK AS SAMPLE. THANKS
