Urgent help pls!

Hi

I have a quick question regarding Photo
1) How many photos required for AN adult application form.
2) so I need to staple / Stick on the Referee page.
3) should it be signed at backside if it is staple in a clear plastic cover.
4) Do I need to send cheque whom name should I write "Home Office" is enough?

5) I have to take returned Special Deleviery envelop to NCS on the appointment day, so HO can return my approval letter in the envelop.

6) I am Director of LTD company and also working part time as an
employee. ( I have no income from Ltd company) so I need to tick an employee only. and same accountant of my Ltd company, can sign as refreee on my application.
