How to fill passport request form

I need to ask both of our passports back (EEA1 and EEA2) I'm a bit confused about how to fill the form...seems a bit complicate...
please help..my mother in law is really ill and I need to get back to italy before something happens :(((
EEA1 & EEA2 sent: 25/04/13
Received by UKBA: 26/04/13
COA (with right to work): 02/05/13
PP return request: 31/05/13
PP returned : 07/06/13
EEA1 & EEA2 : 22/06/13 (issued 17/06/13)

Like this option.. Home office (HO) reference or fee reference* – Nationality only...
I left it blank in the end...
EEA1 & EEA2 sent: 25/04/13
Received by UKBA: 26/04/13

If you have received a CoA with a case ID then enter it there. If you do not have one then yes, leave blank.

I just sent them a first class mail ...still no answer... :(( Im really depressing now..
