Dependent Visa Confusion online application Section5 Tier 1

Hi there ,

I am going to apply for Tier 1 General Dependent Visa for my wife i am facing biggest confusion while applying online . Section -5-Immigration.

* My PSW was refused in 2009 including my dependent (Wife)
* Visa was refused because my bank statement went down for couple of days.
* We left UK voluntarily as our appeal was Dismissed By AIT Judge

* I Applied PSW Visa For My self from pakistan and planed My Self settle first as i lost my job as well and then i will apply my Wife's Visa Later

*Got PSW Nov 2009 .

*Applied For HSMP in 2011

* Got HSMP Feb 2012

* Now i am applying for my wife's Visa as Tier 1 General Dependent .


Please Guys Help or Advice me in this section

Section 5 - Immigration History
What type of notice was it

= Deportation Order

= APP104

= IS151A

As i mentioned early i left UK voluntarily i never received any of these Notice and I can't Esc This Section .
i have tried every where but no luck . so you guys are my final Hope
