Questions about FLR (M) application! Please help...

I received my ILR BRP card today.. and now am preparing for wife's FLR (M) application...

1. Can somebody suggest what documents we have to submit?

2. Also that - about english language requirement.. will it be okay if just show her bachelors' degree-certificate from India (that is in English)? Or she will have to give any English exam? If so, can she just give Life In UK test?

3. By when do we have to apply for her FLR M application? (Her current Tier 1 dependent visa expires in 2015)
