ILR Dependent application query .. pls help

Currently i am in tier 1 visa and next month iam going to apply my ILR. Got married last year and she came here last December. Please clarify

1. what type form of application form i need to fill for her? is it PBS Dependent form? How many years the visa will grant her?
2. In same ILR poe appointment will they allow me to submit her application form also? or else i need to take separate appointment.

Yes PBS dep extension assuming your ilr is based on PBS and yes it can be done at the same time,
