PIO application at CGI Birmingham

Myself and my wife are on ILR and we're about to apply for our 3 month old daughter's PIO card at CGI Birmingham. Can anyone please confirm:
(a) the number / size of photographs required?
(b) What needs to be filled in for 'Permanent Address' ? we are in rental accommodation and don't have permanent address in the UK
(c) If thumb impression will do for the final declaration as well or do we (both parents) need to sign?

Would be grateful for any help on this as the CGI Birmingham website is confusing at best.
