Which form to use? Tier 2 (General) or Tier 2 (W) in General

Hi,

I have been on PSW for 2 years and then on Tier 2 for 3 years. Now, I am sending my application for extension but I am not sure which form should I use?

Do I have to apply with Tier 2 (W) application as shown in the below link:

To qualify for the additional leave, you must have previously been given permission to stay for less than 2 years as:

a Business and Commercial work permit holder (which includes ICT work permits);
a Sports and Entertainment work permit holder;
a Jewish agency employee;
a member of the operational ground staff of an overseas-owned airline; or
a minister of religion, missionary or member of a religious order.
PSW isn't on that list - so that's not the right form.
