Type of Application Form to Use for ILR application

I will be applying for my ILR based on 10 years soon, 8 years on student
visa and 2 on post graduate visa. Please advise me on which set of form i should use and the fee needed.

I have done my Life in uk test and have got two passports that adds to the 10 years stay. Just wondering if i need to provide other documents?

Do they need prove of address for the 10 years?
I have just moved to a new address which i intend to use for my application but i dont have any documents that link to it for now.
