Tier 1 General ( Dependent - Deceleration )

Hi There

I am currently filling Tier 1 General Dependent form for my 4 Months Son.

at the end there is page of Deceleration where it says to add:

Name:
Signature

can you please advise me what should i write in "Name" and "Signature section"

Should i write my Son's name in "Name" Section at the end of Decleration page or Should i write my Name in there as a Main Applicant ?
