urgent help please for tier 2 visa application...

hi there,

on 17th july i am going to apply tier 2 visa in person service. now it will be greatly appreciable if anyone pls advice me which documents i should submit with application form and which order i should attach my all documents pls. please bear it mind i am switching from tier 4 visa.

please help me about that please please please....
