Newborn baby dependent

I am filling up Tier 1, 2 or 5 Dependent Application form for my newborn baby boy; its says in:

K10; How did you enter the UK? (ht)
By air
By sea
By the Channel Tunnel
Travelling overland via the Republic of Ireland

Should I leave it blank as my son born here.


M13; You must provide evidence that you have access to available funds in order to support yourself. Please tick the relevant box to confirm what evidence
you are supplying:

Personal bank or building society statements covering a consecutive 90 day period


Well, my son is just 4.5 months old therefore I am sending my personal bank statement, should that be alright?

On the declaration section it says:
If you are under 18, your parent or legal guardian may sign.

Means can I just write my baby name on name section and sign by myself?
