Home address query

Hi all

I had a quick question regarding the Form AN.

My home address (where I reside) is different to my bank statement/drivers licence/electoral address (which are all registered to my uncle's address). Which address should I include as my home address on the form? I used my actual home address when applying for the IDLR.
