As long as you have your mother's indian passport with your name on it, you should be fine. I dont have a surrender certificate or any other documents of naturalisation. I only have my Indian Birth certificate. My father also has an OCI card.
Will I need to provide a lawyers' Affidavit?

If you do not have the original indian passport but just a copy then you will need an afidavidt.
