OCI Application -Born in India but never had Indian passport

Hi

I was born in India and moved to the UK when I was a minor in the 1970s under my mother's Indian passport as my father acquired British citizenship before I came to UK.
I am now applying for my OCI and confused what documents I need to provide.
I have an Indian Birth Certificate so do I also need to provide the original Indian passport my mother used which has my name added on it when we moved here?
I have never had a naturalisation certificate.

Also, what would I put under the Acquisition of present nationality section:

Acquisition Method? Is that Naturalisation or Decent?
Acquisition Date? Date I arrived in UK or Date I was born
Previous Nationality? As I arrived in UK under my mother's Indian password, was I officially an Indian till I arrived?

I dont have a surrender certificate or any other documents of naturalisation. I only have my Indian Birth certificate. My father also has an OCI card.
Will I need to provide a lawyers' Affidavit?

As long as you have your mother's indian passport with your name on it, you should be fine. As long as your mother's passport that has not been surrendered for over three years since taking up foreign nationality and the passport expired on or before 31.12.2004 no surrender is required.

As long as you have your mother's indian passport with your name on it, you should be fine. As long as your mother's passport that has not been surrendered for over three years since taking up foreign nationality and the passport expired on or before 31.12.2004 no surrender is required.
