Dependent (Wife's) ILR / Extension and English Test

Hi All,

I have been granted ILR on 10 years basis.
My mrs entered into uk and visisted home as per following:

1. Entered in 09/2005.
2. Then visited home in 12/2005 and then she came to uk in 01/2007.
3. Then she went back in 01/2008 and came to uk in 11/2009.
4. She visited home 02/2012 and came back in 09/2012.

I have following questions:

1. Is she eligible to apply for ILR as my dependent?
2. If not can she apply for extension as my dependent?
3. In both cases does she need to appear in English Test?
4. In both cases does she need to appear in "Life in UK Test"?
