Need advice on HSMP application from USA

I am planning to apply for HSMP soon, once I receive my birth certificate and English-language instruction proof(from India).

I am planning to use my W-2(statement of earnings) for 2006 as proof of income along with my payslips. I guess I have to apply before end of March( before the 15 month period elapses). Can somebody please verify this ? I want to avoid having to use bank statements or other letters as proof. What I see from the posts is that these are error-prone and not objective. Since my W-2 only covers Jan-Dec 2006, I guess I have to hurry. Am I thinking right here ?

My original degree certificate has only my initials and not the complete last name. Is this going to be an issue ?

My pay-slips are mailed from my company HQ but they are not on the letterhead. Is this a problem ? They are not online printouts though.

I appreciate your help on this.
