Delays in EEA2 Application

I made an EEA2 application on 12 Jan 2013 and UKBA sent Certificate of Application Letter on 12 Feb 2013. This letter clearly states that my application would be decided within 6 months from date of application.

In April 2013 UKBA sent a letter requesting I and my wife to attend an interview scheduled in May 2013. I informed the UKBA that we would not be able to attend the interview because my wife was 9 months pregnant at the time and could go into labour any time (evidence attached with reply). However,I invited them to come and conduct the interview at our home.

To date, 13/08/2013 I have received no further correspondence from the UKBA.

About 2 weeks ago, I contacted their helpline and was informed that my application status remains "pending decision".

About a week ago, I sent a letter asking for reasons for delays in making decision and enclosed our son's birth certificate with other family photos in the letter.

Still no correspondence and I now want to issue a Pre-Action Protocol Letter. I have seen the guidance on how to write this letter. However, I want to know whether anyone here is
1. having or have had similar experience
2. How they dealt with it
3. whether anyone can share a template/sample of Pre-Action Protocol Letter.
