Help! Have to apply for entry clearance this week!

Hello,

I was granted a HSMP work permit 5.5 months ago. So I need to apply for my entry clearance now, but I'm woefully unprepared, and the official sites I have found have no concrete info.

Can someone direct me to specifically what documents I need to submit with my application for Entry Clearance?

But it says nothing about bank statements, etc.

Do you know if USA citizens need a medical certificate?

Also, once they receive my application, how long will it take to get my passport back? If I need it to travel, will they expedite my application?

Also, I don't know what date I will move--do I have to give a specific date? I'm going to try and hold off until the last minute, so 6 months, I guess.

I've also heard that you can ask them to post-date your visa to give you an extra few months. Is this true?
