Can someone please advise what form should be completed to get into Ireland for non eea family member in uk.
Some people talk about an online application. If this is correct can you point me to web page.

There is no such thing as a family permit in Ireland. You can apply for a visa to accompany an EU family member (free of charge and only requiring your passport, the passport of the EU citizen, a proof of the family relationship, and a letter from your family member to confirm you will be travelling together). The visa application website is here: https://www.visas.inis.gov.ie/AVATS/OnlineHome.aspx

If you satisfy the requirements, there is also an option to travel without a visa under EU directive 2004/38.
