ILR APPLICATION FOR PBS DEPENTENT

I am a PBS dependent living in UK more than 5 years. My wife got her ILR 3 months ago. Unfortunately I could not apply together for my ILR. Which application form I need to use now to apply for and how much the fee. It is very confused in UKBA site...

Did your spouse apply as a PBS applicant or long residence applicant?

Yes ... PBS applicant..and she got ILR 3 months ago
