Tier1 main applicant eligible for PR But dependant not

Hi Gurus,
I am in UK on Tier1 visa (Leave to remain). My wife got my dependant visa in March 2013 and joined me in April 2013.In December 2013 I will have 5 years completed in UK and eligible for PR. Until December my wife only complete 8 month in UK so she does not qualify for PR in December.

1)So which visa form I should be use for my wife?
2)Does she need IELTS?
