applying for naturalisation of BC spouse.

Hello,
briefly, i will tell about my background. i am married to an EU national in 2007, my husband naturalised last year in September 2012 as a british citizen, but still has not applied for his bristish passport. he has his naturalisation certificate. Now, i have applied for EEA4 and i have obtained my 10 years visa in March 2013.
i have taken my Life in the UK test and i want to apply for my naturalisation as spouse of BC before October 2013 ( since the LIUK test will soon change).
my question is when i fill in the AN form, do i have to put details for 3 years or can i go back to 5 years?
also, what are the supporting evidences: Marriage certificate, my passport, husband's naturalisation certificate, letter from my employer....
do i have to send evidence that my husband is working and do i have to show evidence of residency like bills/bank statements....
please, kindly advise me.many thanks
