Surname Change on Indian Passport

Hi

I have recently got BC and my wife is on dependant (Indian nationality). She is planning to travel to India next month for delivery and our baby is due in Jan'14. Currently she has got her maternal surname in her passport. I would like to change her surname and need some advice regarding this.

1) Is it better to change the surname before baby arrival? or can we change later also.

2) If we change the Surname after baby birth, baby Birth Certificate will have different surname. Is this is a Problem?

3) If we change surname in India, are they going to issue new passport or amend existing one?

4) If new passport issued, can we carry the visa on existing passport? or do we need to apply visa separately.

Kindly please advise.
