Biometeric Card- Marriage Certificate

Hi,
I and my wife hold ILR since June, 2012, my wife wants to retain her Indian Passport. She has not changed her surname since marriage ( 4 years back) but now wants to change her surname. We have marriage certificate dated 4 years back, can we use for change of name in Biometeric card as it is asked in the section 6?

Also in the section B.11 and B.12 regarding the national insurance which option should we use as we have not informed HMRC?
