Dependant visa info needed

Hi,

I am planning to apply for dependent visa of my newborn from India (Delhi) though he is born in UK.

Please provide me following information:

1) Which application form to fill up? I cannot find dependant visa option in online application portal of VFS..so is it paper form?

2) What is the fee and do they accept card payment?

3) What are the mandatory documents apart from below?
i. Baby passport
ii. Father passport and UK BRP card
iii. Mother passport and UK BRP card
iv. Baby birth certificate (UK short+long and Indian High Commission)
v. UK Bank Statements (12 months?)
vi. UK payslips (12 months?)
vii. UK P60
viii. UK Rent Agreement
ix. UK Utility Bill
x. TB certificate

4) What are good to have documents?

5) Any other information I should have
