Documents for Tier 2

Hi, I received COS from my employer yesterday which has been issued last week. Now I am planning to apply for Visa to switch from PSW to Tier 2 within 2 weeks. I am planning to submit following documents with online submission copy:

1. Photo
2. Passport
3. Bio Metric Card
4. COS letter
5. Degree Certificate
6. Degree Transcript
7. 3 Months Bank Statements ( though my sponsor tick maintenance for me, I am sending bank statements to cover dependants )
8. Marriage Certificate ( for dependant )
9. Birth Certificate ( for dependant )
10. Prepaid Next Day Delivery Envelope
11. A covering letter from myself
12. Last 4 Weeks payslips

Did I miss anything ? Do I need to send a letter from employer as well ? The director who is the main and only contact person with the Home Office went holiday yesterday for 1 month. I can get a letter from company's other director. Should I get that letter or is it ok without the letter ? Or anything else....

Please experienced guys let me ASAP...

Hi, Anyone ? And, also I was checking application form, there is a question about previous Home Office Reference numbers.... I wonder what kind of reference number they asked.... Previous visa extension case number, or reference number they stated after they received my previous application. Or should I keep blank.... Little confused.
