Appointment for SET (O) [ILR]; No online payment

Hello friends,

Sorry, if this query is repeated but I am wondering if anyone has been in this situation before?

I scheduled a PEO appointment for SET(O) application for 1 dependent and myself on the UKBA website in July 2013, for which no payment was made (No payment was required to book an appointment then). I later re-scheduled this appointment to Sep 2013.
Now, I have the appointment letter with me but no payment has been made online.

Does anyone know if I would be allowed to submit my application on the appointment date?

Another silly question - I am showing personal and business bank stmts as earnings proof (LTD company). A statement for a specific period is missing, and I have printed this online and is stamped by the bank officer. The problem is that the new statement is on TSB letterhead, as my account is moved to TSB from Lloyds, while all the other bank stmts are on Lloyds letterhead. Would this be acceptable? Lloyds is unable to issue me any statements.

Can the form and original supporting documents be hole-punched and submitted in a folder, just to avoid presenting in a shoddy manner?
