ILR Query on Form [Seniors/Gurus.. Please Help]

Hi All,

I am applying for ILR in 2 days time and i have done all the payment online, so do not want to take any chances hence need all your help:

I got my Tier 2 visa issued in 2008 October 26 and i came to UK on the same day, I have switched to Tier 1 in Feb 19 2010 and extended in Sep 2012 for 2 years, so i have the current extension till September 2014.

I have 2 queries

1) I am using 1 SET(O) form for the application and i have added my dependents details on the same form (wife and 1 kid (4 years old)). Should i have 1 form for all of us or should i use 3 forms ?

2) I am applying ILR on October 3rd which will fall within 28 days of my 5 years completion date (2013 October 26), is the application date correct?

Kindly do reply as i have very limited time and since i spoke to a friend i am a bit confused.
