SET O and Dependant -Urgent

If some one is applying on the basis of 5 years Tier 1 General along with Dependant wife he has to fill SET (o). but for Dependant does he need to fill up any separate form or just need to include in his application?
somebody said to me PBS Dependant form????

They apply together it is an extension that has a PBS dep extension form.

my friend is actually applying for ILR on the basis of 5 years under Tier 1 (General).His wife is with him since 2010.He has filled the SET O and included his wife as dependant but someone confused him that he needs to do PBS for his wife .
can you please confirm if he need any other form for wife???
