Tier 4 Dependant Extension in Country

Hey,

I am about to apply for my Tier 4 visa online at the same time with my wife who has a Tier 4 Dependant visa.

I am little bit confused whether she needs to fill a separate form( Online or Paper based?) or I need to include her in my online application or not?

In the UKBA guidance there is a section saying:

4. A family member of a Tier 2 and Tier 5 PBS migrant applying in the United Kingdom can make
their PBS (Dependants) application online or using the equivalent paper application form. Family
members of a Tier 1 and Tier 4 PBS migrant applying in the United Kingdom must make their
PBS (Dependants) application using the paper application form.

Does this mean that she has to fill the paper version (traditional way)? If so, how can I find that printable form? It is not there anymore.

Could someone please clarify this?
