SET(O) form filling queries

My husband (main applicant) and I (dependant) have an in-person appointment for ILR at Croydon. The basis for ILR is 5 years owith Tier-1 General.

(1) In the payment details section (page 4) do we need to fill payment details? In specific questions 6, 8, 9, 10, 11, 12? We have already made the payment online via credit card while booking the appointment.

(2) Does '6.11 Please give the dates of any absences spent outside of the Crown Dependencies and the UK' have to be filled? This will be a repeat of '6.2 Since then have you had any absences from the UK? If yes, give the dates you left and returned to the UK and the reason for the absence in the spaces below'.

(3) For 7.12 can you please give an example of what needs to be filled in the column 'Social cultural or family ties'? This is for country of birth, country of nationality and any country where we lived more than 5 years.
Do we need to include UK as well?

1. No need to fill payment page, attach PDF receipt from world pay confirming payment.
3. I mentioned 'I have family' for home country and included UK as we'll stating 'I worked here for last 5 years'
