spouse visa from pakistan

hi, my wife is 60 years old and has dementia.
Is it true she is exempt from English language requirement for the purpose of spouse visa?

Does anybody know what sort of evidence would need to be provided. she has certificate from her doctor. Does she need anything else like scan/x-ray etc etc
