ILR - Set O form questions

Hi,

I am a bit confuse on few question in the form. I would really appreciate if someone help me understand and answer them.

I have been in UK since 9 year. came as student - PSW - Tier 1(general)
Going to apply in couple of weeks time.
Switch to Tier 1G in UK in 15 May 2009. Applied for extension in May 2012 got BRP in 21 June 2009.
Can I apply in april.
For evidence about out of country it is going to be difficult to get a letter from my employer as I have never inform them that I am going out of country. I have just taken annual leave. Will a letter stating my annual leave entitlement for each year be sufficient if it also states that I have never had exceeded them.

What should be the answer for:
Q6.1 - Date for when the visa was issued in UK
Q7.11 9years or 4.11years
Section 10
Q2.3
Q2.4
Q2.5
Q8
Q9

Section 11
Q11L - Since my last 5 years are made of only Tier 1 (general) - Do i still need to tick this option.
