Tier 2 Priority service or premium service

Dear Friends ,
I am planning to apply for Tier 2 General visa .Due to some reasons I need it asap .so looking for options ike priority service or premium service .

1 Can Individuals by themselves apply directly apply for Tier 2 Visa priority/premium service or is it mandatory that we approach through a UKBA/GOV approved Immigration Adviser inorder to avail this priority service or premium service .

2 I have the following following link to Tier 2 priority Application form https://www.gov.uk/government/uploads/s ... _04-14.pdf
On the front page it states " You also need the separate Tier 2 of the Points Based System - Help Text and Policy Guidance for this form. " What does this mean ? do I have to also fill another form what you call as Normal Tier 2 visa Application form ?

3 Are there any other special docs or Police registration or any other stuff which we need to submit inorder to apply through this priority or premium service .As long as I satisfy the required points is that not enough ?
